TERRA | ENHANCED ASSISTED SENIOR CARE
Terra is designed for residents who require higher levels of physical assistance and care, including certain intermittent skilled nursing services. The Inspīr lifestyle, including our Nine Core Elements, is thoughtfully adapted for Terra residents to meet their specific needs, abilities, and goals.
Assisted senior care is highly coordinated by a dedicated Care Manager, and Terra residents benefit from higher staffing ratios, 24/7 on-site nurses, on-site physical and occupational therapy, on-site medical services, integrative health and wellness programs, and access to all of our life-enrichment experiences. Terra includes personal care, medication management, and some skilled nursing services, and all Terra residents receive comprehensive assistance with activities of daily living.
Terra residents and their families also benefit from Inspīr’s Integrated Care Model (ICM), an interdisciplinary team-based approach to whole-person health and wellness. ICM synthesizes the expertise of professionals across multiple disciplines for the best possible care experience and outcomes. Care Managers help residents navigate more smoothly through care transitions, coordinate care across team members and outside providers, and keep family members informed along the way.
CARE SETTING
While Terra residents have access to all amenity spaces in the building, they benefit from a smaller, more intimate living setting with higher staffing ratios and dedicated care partners assigned to each floor. Each Terra floor is purposefully designed with 12 apartments and a beautifully appointed common living room, dining room, and open kitchen, promoting social engagement and a sense of community.
Smart home technology allows residents to control lighting and temperature, or call for assistance using their voice. And the same smart wearable that residents can use to call for help also unlocks their apartment door.
CARE TEAM
Each Terra resident has a dedicated Care Manager who ensures care and service plans are individualized and aligned with the resident’s abilities, choices, and goals. Assisted senior care plans are designed to proactively avoid unnecessary emergency department visits and hospital admissions. Care Managers keep families informed and skillfully assist with care transitions and care coordination across providers, settings, and time.
Our interdisciplinary team, including nurses, social workers, home health aides, physicians, mental health providers, pharmacists, nutritionists, physical and occupational therapists, palliative care and hospice specialists, and wellness experts all work in close collaboration to attend to the more complex care needs of Terra residents and to optimize well-being using best practices and evidence-based approaches.
Through provider partnerships, Terra residents also have access to other on-site services, including podiatry, mobile imaging, phlebotomy, and home dialysis.
For individuals living in skilled nursing facilities who don’t require 24/7 skilled nursing care or medical monitoring, Terra may be an optimal lifestyle alternative.
